How do I get a RenValue card? Not the coupon. I tried using the coupon (from my dialysis clinic), but my Kaiser pharmacy will not honor it. I Had my MD write an Rx to take to an outside pharmacy and was told my co pay would be $600+ with the coupon. My Dialysis clinic now gave me a p.s.k.w. & Associates form to complete, but I am not able to complete without a RevValue card # or group #.
I tried using the coupon (from my dialysis clinic), but my Kaiser (Santa Clara, CA) pharmacy will not honor it.
We also applied for financial assistance but haven’t heard back in 2 weeks.
Please advise.
