I received my $12 rebate check (#737331, dated 04/29/2013. The name on the check is misspelled. It should be “Gloria Homer” but instead is “Gloria Horner”. Obviously I can’t cash it. Please send me a new check. Thank you.

My mother received the Visa reward card for $12, however, no one will let us use it. it won’t process like a gift card and when they try it like a debit card it needs a password. She doesn’t have a password for it. It is just sitting in her purse.
Have you tried the first four number of the card or the last 4 numbers of the card as you password? I think that is what I had to do when I used my Visa reward card for 12 dollars

Heartgard used to have a $4.00 rebate form for the purchase of 6 doses and the $12.00 rebate form for 12 doses. I no longer see the $4.00 rebate forms. Is this still a viable option and if so where would I get the form? Thank you!
