I too cannot get a rebate form. My dog has had 6 injections and 1 more tomorrow from the vet so I don’t have a code to enter.
my name is Betsy Schrauth and I have entered my dogs information on the website but am unable to get a code or coupon code to enter. I use the Veterinary Clinic of Myrtle Beach Myrtle Beach, SC 29579. Seems like I am not the only one with this problem.
